Okay, adding comments here. It's an illustration done completely in Photoshop with nothing more than a mouse and a regular brush. Trying to teach myself a few new things here. Thanks for the comments thinking it was a photo. Guess I'm kinda flattered at that!!

Holy moly... that's not a photo? I'm... I'm moved!

The miracle of simplicity is readily apparent here. You've done a wonderful, wonderful job of making it look photorealistic; the "lighting," as has been commented on before, is fantastic. I especially marvel at "her" neckline and hands; they're extremely convincing. Definitely the level of professionalism I'd see in an ad... probably for a hospital, or perhaps even for teen pregnancy. Shrug It has a somber, artsy feel to it.
